Describe the normal everyday pattern
Focus on what support was needed, what risk existed, and what would have happened without help.
Describe whether help was needed to sit up, stand up, sit down, reposition, or transfer, and what risk existed without help.
Include balance, pain, dizziness, holding furniture, walking aids, someone guiding, or risk of falling.
Include physical help, supervision, fear, breathlessness, pain, or avoiding stairs because of risk.
Include falls, near-falls, stumbling, slipping, being caught, or needing supervision to prevent a fall.
Write one example: what movement was difficult, what help was needed, and what risk existed without help.
Write down confusion, memory problems, orientation issues, unsafe decisions, and support needed.
Include recognising familiar people, carers, rooms, objects, or situations.
Include time, place, people, routines, getting lost, or needing repeated reassurance.
Include unsafe choices, forgetting steps, poor judgement, or needing someone else to decide.
Include repeated questions, not knowing where they are, mixing up day/night, or unsafe confusion.
Write one concrete example of confusion, memory difficulty, unsafe decision, or support needed.
